Rinse it in old tank water every 2 to 4 weeks, never under the tap, as chlorine in tap water kills off the beneficial bacteria your filter relies on.

It depends on the type. Mechanical foam can usually be rinsed and reused for a long time, biological media should be kept in place and only replaced once it's genuinely falling apart, and chemical media like carbon or phosphate pads needs replacing regularly once it's exhausted.
